Mounjaro (tirzepatide) is a dual GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) receptor agonist.  Ozempic/Wegovy (semaglutide) are glucagon-like peptide-GLP-1 produced in the gut. Mounjaro and Ozempic are diabetic medications, while Wegovy is only FDA approved for weight loss. All have been found to cause weight loss in those taking. Thus, it is becoming widely used in patients with or without diabetes for weight loss because of its effects on the body. They affect glucose control through several mechanisms; slowed gastric emptying, reduction of postprandial glucagon and food intake. They are a once weekly injection administered subcutaneously.

No particular meal plan is required, we recommend utilizing a plan that has worked for you in the past or as you begin lifestyle changes in your eating habits, making healthy choices and “clean” eating. These choices will help you balance your future lifestyle and maintain a healthy body. Exercising daily is recommended or at minimum 3-4 times weekly. Mitocore multivitamin is essential nutrition replacement. During your weight loss program we recommend/require this nutritional replacement to aid in your overall well-being.

Side effects

Most people taking these medications experience few effects. However, the most common side effects include, but are not limited to; nausea, diarrhea, vomiting, constipation, indigestion, and abdominal pain. These side effects are typically mild and resolve quickly. They may also be more likely to occur when eating foods that are fried, greasy, fatty, or high in sugar because these foods are harder to digest.

Hypoglycemia (low blood sugar) is also a potential side effect when taking Mounjaro, although the risk is small because of the dual action of this medication and its tight glycemic control.

Other, rare, but potential side effects include, but are not limited to, kidney problems, gallbladder problems, pancreatitis, or changes in vision.

Nausea/Vomiting – take Digestzyme-V -Take 1 capsule as needed

Constipation– Super Aloe – take 1 capsule daily as needed (results usually within 24 hours). May take as needed or may take 2-3 times weekly to maintain regular bowel movements. If constipation persists may take Motility Pro.

Indegestion – may take Pepcid as needed

If you experience mild side effects and they resolve that is to be expected, if they do not resolve quickly, notify our office or if Severe/Serious side effects seek Emergency medical attention immediately and stop medication.

Who is NOT a candidate for Mounjaro/Semaglutide?

Anyone with a history of the following:

Thyroid C-cell tumors: Medullary Thyroid Cancer (MTC), Family history of multiple endocrine neoplasia syndrome type 2(MEN2), Diabetic retinopathy, Type 1 diabetes, Pancreatitis, Active gallbladder disease, gallbladder problems, or gallstones, Kidney failure, Gastrointestinal issues such as gastroparesis, Family history of Medullary thyroid cancer, Pregnant, breast-feeding, or trying to get pregnant, Heavy alcohol consumption, Allergic reaction to Mounjaro or any of its ingredients, Any other significant medical history will require clearance from their PCP

Who is eligible and how does the weight loss program at Dermahealth work?

When taking medications for weight loss, it is recommended that a patient have a BMI of 27 or greater. Mounjaro for weight loss is considered “off label” at this time.

Once we have determined that you are a candidate and you have decided to enter the program, we will discuss your weight loss goals, healthy eating habits, and obtain your vital signs, weight, height, BMI, and waist circumference. Your blood will be drawn prior to starting the medication to check your baseline kidney function, liver function, electrolytes, thyroid function, hemoglobin, and diabetes markers. Once we receive your lab results and there are no concerning findings, you will be ready to start the medication for weight management with Mounjaro. Once per month CALL OUR OFFICE and speak with a Practitioner to discuss your progress and your next prescription will be sent at that time based on that information. You will be asked to return to Dermahealth every 3 months for labs and meet with a practitioner to discuss your diet, exercise, lifestyle changes, and any concerns or questions you may have regarding your progress. During this process, the hope is that you will have attained your weight loss goals through lifestyle changes that include healthy eating habits, exercise, and with the assistance of medication.
